Currently, she directs employee well-being programs for the 8,000+ staff in the Mid-Atlantic States region which have achieved Gold Status through Healthiest Maryland Businesses for two consecutive years and has recently been appointed to interim director for equity, inclusion, and diversity. Previously, she directed the Regional Health Education, Health Promotion, and Women’s Health team and led the development and implementation of programs to improve member engagement, lifestyle behaviors, and health outcomes. Shannon holds a MS in Health Promotion Management from American University and a BS in Exercise Physiology from Howard University.[/vc_column_text][/vc_accordion_tab][vc_accordion_tab title=”Gary Puckrein, PhD – National Minority Quality Forum”][vc_column_text]NMQF CEO Gary PuckreinGary A. Puckrein, PhD, is President and Chief Executive Officer of the National Minority Quality Forum (formerly the National Minority Health Month Foundation), a not-for-profit organization that he founded in 1998. The Forum addresses the critical need for strengthening national and local efforts to use evidence-based, data-driven initiatives to guide programs to eliminate the disproportionate burden of premature death and preventable illness for racial and ethnic minorities and other special populations. The goal of the Forum is to strengthen the capacity of local communities to eliminate the disproportionate burden of premature death and preventable illness in minority populations through prevention, early detection, and control of disease complications. Dr. Puckrein also serves as the Executive Director of the Alliance of Minority Medical Associations (a collaborative effort of the Asian and Pacific Physicians’ Association, the Association of American Indian Physicians, the Interamerican College of Physicians and Surgeons, and the National Medical Association), which was formed on the basis of evidence-based data that highlight significant disparities in health-care treatment plans and outcomes among many underserved populations within the United States. In April 2001, the National Minority Health Month Foundation launched National Minority Health Month in response to Healthy People 2010, the national health-promotion and disease-prevention initiative.[/vc_column_text][/vc_accordion_tab][vc_accordion_tab title=”Basim Khan, MD – Neighborhood Health”][vc_column_text]Basim-Khan-2Dr. Khan joined Neighborhood Health as a primary care physician in 2011. He subsequently served as Medical Director before becoming Executive Director in 2015. Dr. Khan’s primary interest is expanding access to high quality, affordable health care in Northern Virginia. He attended the University of California Los Angeles for medical school and completed his internal medicine residency at the University of California San Francisco in a primary care track for the underserved at San Francisco General Hospital. He received his Bachelor’s Degree from Brown University and a Master’s in Public Administration from Harvard University’s Kennedy School of Government as a Zuckerman Fellow. He served as a Presidential Leadership Scholar in 2017 and was selected as a Washington Business Journal’s nonprofit CEO/Executive Director of the Year in 2018. He has served on the Board of Directors of the National Physicians Alliance and the Culmore Clinic as well as the Governor’s Council of the American College of Physicians. He also has an interest in health policy, health care delivery reform, and economic development. Dr. Khan speaks Urdu/Hindi, Pashto, and Spanish. He is Board Certified in Internal Medicine and is a Fellow of the American College of Physicians.[/vc_column_text][/vc_accordion_tab][vc_accordion_tab title=”Connie Hwang, MD, MPH – Alliance of Community Health Plans”][vc_column_text]Connie HwangDr. Hwang is a general internist with expertise in quality measurement and population health. At Alliance of Community Health Plans (ACHP), she is Chief Medical Officer and Director of Clinical Innovation, providing clinical leadership and shared learning opportunities on innovations in health care transformation, provider partnerships and quality improvement. Prior to ACHP, Dr. Hwang was Vice President of Quality at Evolent Health and led improvement initiatives for Medicare Advantage Star Ratings, CMS ACOs, State Medicaid quality programs, payer partnerships and NCQA HEDIS performance. Dr. Hwang was previously Vice President at the National Quality Forum and launched the Measure Applications Partnership, a public-private collaboration convened to review performance measures for CMS’ value-based care programs.
